PE: $50 because I got a refund on a medical bill that I must have overpaid? I have no idea because I didn't request a refund or anything but they just sent me the money and it says "refund" on it.

With good orgs, you don't have to request a refund. I've received non-requested refunds from both the VA (overpayment for my meds), and just recently, from the Treasury Dept. (and I have no idea why I got that one).
