You can use your worker ID as the code you submit the hit with, and then contact them telling them what's up.

9 times out of 10 you'll get approved. On TurkPrime surveys, most of them are set to reject instantly if the wrong code is entered but they'll usually reverse it for you after you let them know about the no code.

Just input your worker id at the top of the mechanical turk page. it automatically copies when you click it. You can email the requester if you like, but I don't really find it necessary. If you didn't get a code, other people didn't get a code, and someone will email the req. and let them know.
