That's a great help.The best example that I can think of is either A9s or MLData (they use radio type buttons on some of them)....I don't see any up right now though......or Ben Peterson (that one is for judging dating profile photos), if you have a worker account, you can see what he has done. I don't know if that format would work with audio, but there are buttons and a small space to write things if you want.

If I was working on it myself, I would make the file auto play on load if visible, focus the text field, and make keys for any radio buttons and submit button. Workers won't mind adding in the extra stuff themselves to make it easier if it pays a decent wage.

I'd have all ten seconds of text above the audio and I'd tell you when your actual HIT text begins (0:25).
Tricky for me: A worker needs to identify which words need changing. So, maybe checkboxes in a tall scroller on the right having a checkbox for each and every word of the HIT, and magically, these words show up below that with a blank space for the correct one to get typed in.

a 10-15 min user test is 10.00. I mostly use usertesting.com, but i also use validately and userlytics.
