If you go to completed HITs in your Dashboard, look for the survey and there should be a Contact Requester button at the right that pulls up a form. It'll include the HIT ID in the message title so you don't need to worry about that. Usually, when there's no code, I'll email them and let them know and include my worker ID, and if I can remember I either copy-paste the text from the last page of the survey or take a screen cap, upload it to imgur, and send them the link to the image.
