You need to wait around 5 second before hitting accept during a captcha. Otherwise it will tell you that you got it wrong.
Or, if you feel like being a rebel, type it in your usual ultra-fast manner, and refresh the page once it tells you that you got the code wrong.

It's rare for me to and I don't remember having this happen for over a year at least. I've cleared approximately 3755 captchas turking (a number which is a bit depressing considering how many juicy HiTS have slipped away while clearing them...)
