I read that and it helped me up until this point. But it is vague on these particular inputs. People who work on Mturk are rare, and i am looking for insight from people who have experience with this specific issue. not tax advice. i'm sure everyone files taxes so at least someone here should be able to give me an answer from experience.

Short answer: You're in the wrong form if it's asking for that information.
Long answer: Read more of the link provided, it's written by turkers for turkers. There is no real good long answer short of walking you through it.
