How did you access the survey?

I just clicked on whatever the link was...it was like one of those survey monkey things, but I'm not sure if it was that exactly. The format was like that though if it's the one I'm thinking of. Did yours not open?

Yeah mine wasn't working in Chrome but I tried in a different browser
