That is the problem, there is no box to submit the URL.

these are all pretty generic but worth thinking about:
1. what OS are you using and has it received all its updates?
2. what browser are you using and is it updated? have you tried another browser and gotten the same issue?
3. for both of the above did updates occur 2 days ago and that's when the qualtrics submission box started not showing up?
4. did you add any new software, ad blockers, antivirus etc 2 days ago? basically was there something that changed on your computer that you are aware of 2 days ago?
I'd do the basic browser resets (temp file clearing, cache/cookies/etc) as well and if you haven't rebooted in awhile do that too. It's odd for a webpage not to load properly but given that it appears this is not a widespread issue it's got to be something local to your computer.
