There's an extension called "EditThisCookie" (Chrome store). Usually when those errors occur that'll take care of it (wipe out all the cookies that are attached to that survey page).

When you have it installed, get to the page where you have the error on the survey (*not* the hit page), click on the cookie in the bar then the trash can. Close the survey page and reopen it through the hit and see if it works now. Hopefully that fixes it for you.
