I forgot if I addressed it yesterday or not, but when you see the "Off" in TO column for HF-MTS while you have TO on. This is because TO failed to respond within 5s, so the request was canceled and it moves on. This was one of the causes of HF-MTS randomly stopping before, the TO request would fail but it wouldn't move on.

That's within the iframe of the HIT and he said earlier that he logged in and out again to make sure that wasn't it.
