Also depends on what scripts you're using since it's a script that puts that there, but both MTurk Suite and the Turkopticon script will add that, if you have either one of those.

I never got MTS but I've always had TO. I only see the "report you experience" options in the search results but not in my daily submitted hits. Mayb I'm looking in the wrong place. Kaduchi's method worked just fine though.

MTS will add TO to the search results, queue, status detail and hit capsule (if reqid is avail)

When I look at the hits I've submitted for a given date, there's only the requester's name, there's no TO button.

The normal TO script has always had that, for me. I'm not sure why it wouldn't be showing up for you.
