I ran through the survey on a VPN to see whether I did actually miss something. I made it to the same page where I failed out the first time. I've got a screenshot of the page with the question and correct answer selected. The requestor has been emailed. I've asked that I get a reply so I can send back proof. Next step is I email the department head if I get no response.
