Wtf, did anyone speak up to him?
Sadly no. Everyone just stood there looking shocked. If I wasn't so far back in line I definitely would have.
Actually, I've found that in that case it can mean that their life is pretty terrible. That's not an excuse for their behavior and he shouldn't have done that, but there is a reason behind everything and everyone. Sometimes, that tantrum is their cry for help, because they're too prideful or scared to ask for help.

It's like this cantankerous bastard of a man that came into McDonald's every morning at my first job. He was terrible. My coworkers would run off, and whoever was left ended up having to deal with him-usually me. I wasn't letting this dude get to me. I was going to use every ounce of cheerful in my body to win him over or at least show him that he wouldn't break me.

Turns out, it worked. Also turns out, he was bitter because he was grieving the recent loss of his wife. They'd been married 50+ years. He also had PTSD, and got freaked out in crowded areas (our lobby was packed in the mornings) but was also pushing himself to get out and do something each day so he didn't sit alone in his grief and loneliness.

Again, not excusing behavior like that. Sometimes, they're just assholes. And sometimes, they need that other person to step in and say, "Sir, ma'am, are you okay?" and you find out they're not angry about the dollar, or other simple thing. Humans be vulnerable, yo.
I agree, most people aren't generally mean, but have other issues going on. Still felt bad for the poor girl though.

You guys I got to witness my first public freak-out on my way home today. An older gentlemen was screaming at the cashier in a supermarket when she asked him if he wanted to donate a dollar to charity. He literally threw a tantrum over $1 and refused to because that was his "hard earned money". Poor girl was in tears by the time I got to the front. I mean, wow. I can't imagine how terrible your life must be to scream in a packed building over an optional dollar.
I worked as a cashier for about 3 months before I found turk, was just trying to get extra money for Christmas and birthdays. You wouldn't believe how many people take out all of their frustrations on grocery store cashiers. I almost got assaulted over a wounded warrior donation. The guy was determined to make some kind of example of me and how dare I ask and a whole lot of other stuff. I tried to tell him that it was my job and I would lose said job if I didn't ask.... he just yelled at me more and longer. I think he was trying to make me cry.... I didn't, until he left the store... then I cried through my next 5 people. They were as shell shocked as I was. The last straw was working on Thanksgiving day and having some ahole scream at me because all we had left were frozen turkeys. That was my last shift. Ever.
