I was born and raised in the South. I'm not really old! For those around me, bless your heart meant you had sympathy for someone. Over the years, the meaning has certainly changed, though.

He don't know what he is doing, bless his heart.
Or she made a mess of that, bless her heart.

or, they're just Yankee's, bless their hearts

it's funny that people online think it's malicious...maybe it's sometimes used sarcastically, but from my experience it's an expression of genuine sympathy.

in my experience it has always been used in a condescending way. as if the user was saying that if the person was a little bit more competent we wouldn't have to be blessing their heart

Same, mainly it's used as a free pass to gossip about someone (which can include insinuating that they are dumb) but still act like you care about their well being.

I got a bonus but I left them open and checked and responded each time I needed to. My HITS were not broken
