I almost never see that and I live in LA County, and closer to never is more accurate (once in ~10,000 surveys). The only time I can remember seeing an error like that was about 2 weeks ago, where lots of other people on TV complained about the same problem. The requester fixed it, and I did the HIT.
This error is specific to Victoria/OCMP requesters.

When i applied for Prolific, they said i wouldn't be able to work for them since my service provider wasn't on their "approved list". However they looked into and added my service provider so i was able to work for them. I wonder if its the same sort of deal with Vicky though.
