I don't think the questions are for the requester you are doing the hit for. They are from any of the Turkprime requesters just tagged on to the bottom of the screen.

What's strange is, I reported the HIT as not being able to submit HIT. When the page refreshed, the PIN request disappeared completely.

Yeah, that's why I think those questions are asked by random mturk prime requesters, because sometimes, I can do one survey for one and then the next survey I do for another requester will have the exact same questions, with my answers already checked. I think Amazon just puts them on there and the requesters don't know if it is on there or not....that is, unless it is right next to where you put the survey code, or above the additional questions.
