If you mean literally every single hit, maybe you should post a screen shot of your HS settings. Or there may be another technical issue.

There are the settings I'm running currently, though I do change it from time to time. I've watched a couple video tutorials and they seem to be in line with what I've seen. It has only been these past week or so that I haven't caught anything. Maybe it's just a spell of bad luck.
