Complete the survey and upload an image of your insurance card. If the survey is approved, you get your choice of $25 in paypal or Amazon GC. After that, they send you a referral link so that you can refer people to the survey. And yes, I make a little bit if you complete it and it's approved.

It took me about 10 minutes on mobile to complete.

Here's what’s required to be eligible for the research study:
