Just don't bee too strict in your choices and you'll be fine. For example if the sentence says "dog" and one of the answers is "dogs" you should check it even though "dogs" implies more than 1 animal. Other examples of things I would check are sentences with the word "children" you could mark words like "small" or "little".

Yes.

Well, that's the key to your being able to prevent this from happening again (assuming you have the $$). You bypass the phone stuff--which is notoriously unreliable--, you pay your bill via a linked bank account (or whatever), and having paid it, you get a verification via eMail.
