Alright, so the problem here is that this task doesn't give you a code, once you reach the end, after it tells you your bonus amount, it submits itself without the need for the code. This is when it all went wrong for you since your mturk logged out on you.

or simply use the new thing that seems to be trending for the requesters - the link forces a full screen takeover of your PC so you can't see or do anything else on your pc but the survey until you end the survey and exit out of the full screen takeover.
