I just wanted to follow up that I reached out to the requester and he is going to manually approve the HIT.

If anyone is curious, this was his reply:

Thank you for your message and bringing this issue to our attention. It seems as though our survey was very popular and many people all began the survey at the same time. This, unfortunately resulted in some people getting the disqualification message after having been allowed to work on the survey. We suspect the issue arose due to a glitch in the survey software.
