check the URL for the survey.. if there's not a ? before the MID= then you need make sure it's a question mark rather than what is there.
so if it doesn't look like this example: https://somecollege.qualtrics.com/jfe/form/SV_6YeDH3L4FS6Oftz?MID=YOURMTURKID
then it needs to (bold and italicized section)

I haven't caught this one yet to be able to be more specific. The above is usually the issue when a survey can't be found.

You might need to allow flash/javascript to run by clicking on the green lock icon in the URL bar if you're using Chrome. Pop-up blockers will make it go funky too.
