I actually just used a custom template one from the internet that looked like a standard form. Explained what the Fax was about, my email address for account, and then had my statement behind it.
I can try something like that if they don't respond to my most recent email soon. This is just incredibly frustrating because my bank account was already verified for my seller's account---not sure why I even have to do it again. But thank you, I will look into a form, do you happen to remember which one you used?
