I just did this survey with no issues. I would contact the requester for your situation.

I sent them a PM and then did another one to show that I'm not a bot nor incompetent. I'm so paranoid when it comes to rejects. I've only ever had one, and that was the requesters fault.

I leave comments on all of them. That's what they ask for, so I'm guessing they appreciate knowing why you're scoring it the way that you are.
