no one knows how masters arrive. Our best guess is that, after you meet a very minimum set of qualifications (hits approved), you are in the lottery . Someone got it on the penultimate go-round when his account was suspended. Others have gotten it under 5K hits approved. Some have waited until they had nearly 1 million.

I would think before Amazon punished someone for doing a HIT that they are hosting, they would just remove the HIT.

they are clearly scam hits - we don't know that the cards are stolen, but we sure think so (there's quite a bit of evidence to suggest that - someone even called one of the people listed and they were unaware of mturk).
