OK, I could use a little help.
I screwed up and submitted a hit before i answered all the questions(forgot to turn off keybinds), the hit had survey questions in the iframe. Req sent me a pdf asking me to fill this in but it is not in a form format so i cant fill anything in and i have no clue what to do with it. Anyone have any ideas for me before I message him back.

I would just compose an email stating that you weren't able to use the form, but here are the answers to the required questions ... and list them out with your answers. I also like to add: "So sorry to have added any unnecessary work on your end".
