I can't speak for others, but in my case it was because some of the times I copied the code, it didn't "take", and I apparently pasted the previous code into the box without realizing.

Not as embarrassing as the few that had "v" as the code because I tried to paste and did it wrong

That happened to me too a few times. Now I always check them to make sure the right one gets entered.

I have done that too! I was confused also because I have had some codes that end with the = sign and some don't.
