I can't speak for others, but in my case it was because some of the times I copied the code, it didn't "take", and I apparently pasted the previous code into the box without realizing.

Not as embarrassing as the few that had "v" as the code because I tried to paste and did it wrong

I know I 've done both of those things, which makes sense of it for me, but didn't think it was that many times, and think I caught the 'v's before sending sometimes just too many windows open and got lost with what was what.
